Requirements Checklists
This is a required form for CMPE students. It must be signed off and returned to Carol Mullane in E2 298H.
Column 1
For courses taken through concurrent enrollment, the student should present an Extension transcript to the Grad Assistant.
Column 2
Submit a transcript and supporting course materials to the appointed instructor. Supporting course materials include syllabus, name of text(s) used (the actual text may be necessary if it is uncommon), assignments, exams and project reports, as determined by the appointed instructor. The approval of a course at another institution for the purpose of satisfying the base requirement does not provide any course credit (units) towards the degree
Column 3
See the appointed instructor well in advance to arrange to take a final exam. The instructor may elect to wait until the next regular offering of the course to administer this exam.

You must obtain instructor's signature prior to submission of form to Sean Tai in E2 298. Please be aware of enrollment deadlines.
Petition for Transfer of Concurrent Enrollment Course
A total of at most three courses (4 in Electrical Engineering) may be transferred from concurrent enrollment (For Electrical Engineering, up to 4 courses are allowed). Once transfer of this course is approved, it's application towards the MS or PHD course requirements will be subject to the same rules as courses taken as a graduate student.
Petition for Graduate Course Substitution
Up to 3 courses (4 in Electrical Engineering) may be transferred towards PhD with department approval. Transferred courses may not be used for MS course requirements, with the exception of Computer Science.
